The Real Ozempic Dose Chart: Why 2.4mg Isn't On It

Is 2.4mg an Ozempic dose? No, here's the mix-up

The mix-up is built into the product line. Ozempic and Wegovy are both brand names for semaglutide. The same company, Novo Nordisk, makes both. Wegovy is approved for long-term weight management. Its usual top dose is 2.4mg. Ozempic is approved for adults with type 2 diabetes. Its pen tops out at 2mg.

The weight side has since climbed higher. The FDA has approved Wegovy HD, a 7.2mg weekly dose. It is for people who have taken 2.4mg for at least four weeks and need more weight loss. None of that changes Ozempic. Its pen maximum has been 2mg since 2022. If a dose above 2mg comes up in a talk about Ozempic, the talk has drifted onto a different drug.

The pen has four dose levels. Each step has a minimum hold time, not a maximum one. A prescriber can keep you on a step longer, but not shorter.

"If needed" matters in that chart. For most people, every step past 0.5mg exists to improve blood sugar control. It is measured by HbA1c, a three-month blood sugar average. The prescriber checks whether the current dose is working before moving up. Plenty of people never go past 0.5mg or 1mg, because those doses already meet their goal. One group is different. With type 2 diabetes and chronic kidney disease, the label moves people up to 1mg as their maintenance dose. At that dose, Ozempic is approved to lower the risk of kidney decline and death from heart disease.

The Ozempic pill dosing chart: 1.5mg to 9mg

The pill has strict rules, because food and drink keep it from being absorbed. Take it first thing in the morning, on an empty stomach, with up to 4 ounces of plain water. Swallow it whole. Then wait at least 30 minutes before eating, drinking or taking other pills.

Pill milligrams don't compare with pen milligrams. Much less of a swallowed dose reaches the blood. So 9mg by mouth is not "more" than 2mg by injection.

What is the highest dose of Ozempic, and does everyone need to reach it?

The highest approved Ozempic dose is 2mg once a week for the pen. For the pill, it is 9mg a day. It is a ceiling, not a target. The label treats 0.5mg, 1mg and 2mg as real treatment doses in their own right. Reaching the top matters only if a lower dose hasn't brought blood sugar where it needs to be.

The picture blurs for off-label use, meaning a use outside the FDA approval, such as weight loss. The label picks doses by blood sugar only. No Ozempic dose has an FDA-recognized weight-loss goal attached. That is why searches for "maximum dose of Ozempic for weight loss" turn up such mixed answers.

The Ozempic pen "click chart": how the dial works, and how many doses are in a pen

Ozempic comes in three pen sizes. Each one is capped at its own labeled dose. You can't dial a pen past it. Each pen is meant to last about a month.

• Starter pen, for 0.25mg or 0.5mg: holds 2mg in total · Four weeks at 0.25mg, then two weeks at 0.5mg · Or four doses of 0.5mg
• 1mg pen: holds 4mg · Four weekly doses of 1mg

To set a dose, you turn the dial, which clicks at each stop. You keep turning until the dose counter window shows your prescribed dose. Some online "click charts" turn that into a click count per dose. They don't agree. One says 8 clicks for 2mg, another says 72.

Novo Nordisk publishes no click chart at all. Its instructions say to trust the numbered dose counter, not a click count. The counter is tied to how much medicine the pen measures out.

Ozempic dosage for weight loss: how it differs from the diabetes schedule

Everything above describes a diabetes label, built around blood sugar. When Ozempic is prescribed off-label for weight loss, the same dose levels exist. What changes is the reason for choosing among them. Some prescribers stop at 0.5mg or 1mg if a patient responds well. Others aim for 2mg on purpose, treating it more like a weight goal. The label covers neither approach, because it was never written for weight loss.

That gap explains why reviews of "Ozempic 2mg for weight loss" are so scattered. Wegovy's weight-loss trials used one standard ladder. Off-label Ozempic has none, so people at 2mg got there by different paths, at different speeds, for different reasons.

How long does titration take, and what if a step doesn't agree with you?

Titration means raising the dose step by step. At the fastest pace, reaching 1mg on the pen takes at least 8 weeks from your first 0.25mg shot. Reaching 2mg takes at least 12 weeks. On the pill, reaching 9mg takes at least 60 days. Many people take longer, and the label allows it. Any step can last longer than four weeks if side effects haven't settled. A prescriber can also step you back down. The goal was never 2mg itself. It is a dose that controls blood sugar and that you can tolerate.

What if you miss a dose?

For the weekly pen, if fewer than 5 days have passed, take the missed dose as soon as you remember. Then go back to your usual day. If more than 5 days have passed, skip it and take the next dose on your usual day. Don't double up. Doses need to stay at least 48 hours apart.

For the daily pill, skip a missed dose and take the next one the following morning. After a break of several weeks, the prescriber decides where to restart, sometimes at a lower dose.

Compounded semaglutide is made by a pharmacy for each patient. It is not FDA-approved. Mochi Health has priced it by dose since August 2026. Is Ozempic dosing any different in perimenopause or menopause?

No. There is no separate FDA dosing schedule for women in perimenopause or after menopause. No Ozempic study has set one either. The same ladders apply at any reproductive stage.

What often changes in this age group is the reason Ozempic comes up. For many women, it is weight gain in midlife rather than a type 2 diabetes diagnosis. That matters for insurance coverage and for how a prescriber frames the dose plan. It doesn't change the doses themselves.
