LillyDirect Zepbound: Price by Dose and How to Order (2026)

The price follows your dose, not the format you pick. Ordering takes a prescription, a text from Lilly's partner pharmacy and one refill habit that protects the lower price.

The short answer

LillyDirect Zepbound self-pay price at each dose

Lilly charges by dose, not by format. A box of single-dose vials and a KwikPen at the same strength cost the same. Each fill is a 28-day supply, which means four weekly doses.

  • 2.5 mg, the starting dose: $299 a fill
  • 5 mg: $399 a fill
  • 7.5 mg, 10 mg, 12.5 mg or 15 mg: $449 a fill, if you refill within 45 days

So the price climbs over your first two fills, then stays flat. Home delivery is free. Needles are not in the box, so you add them at checkout or buy them elsewhere.

Order Zepbound from LillyDirect
Lilly's own pharmacy service. About $300 a month at the starting dose, vial or KwikPen.
See LillyDirect prices
  • Paying cash: LillyDirect, or the KwikPen at a local pharmacy for the same price.
  • Your plan covers Zepbound: the Savings Card, as little as $25 a month.
  • On Medicare Part D: the GLP-1 Bridge, $50 a month if you qualify.
  • You want a prescriber and support in one place: Ro, Joi or LifeMD, with their fee on top of Lilly's price.

Is LillyDirect legit? Who fills and ships your order

Yes. LillyDirect is Eli Lilly's own direct-to-patient service, launched in January 2024. Lilly makes Zepbound, so the medicine comes straight from the maker's supply chain.

LillyDirect is not a medical practice. It does not decide whether Zepbound suits you. A licensed prescriber makes that call, then sends the prescription to LillyDirect Pharmacy.

Lilly then routes your order to one of four licensed partner pharmacies. Which one depends on the medicine, how you pay and whether you want delivery or pickup.

  • Amazon Pharmacy
  • Fuze Health
  • Gifthealth
  • Walmart Pharmacy, which also handles free pickup for the KwikPen and the vial

Lilly sets the same price at every partner. The pharmacy that texts you does not change what you pay.

How to order Zepbound from LillyDirect, step by step

  • Get a prescription. Your own clinician or a telehealth service can write it. The next section covers who.
  • Have it sent to LillyDirect Pharmacy. Prescribers can send it electronically, by phone or by fax.
  • Switching pharmacies? A prescription at another pharmacy does not move over. Your prescriber has to send a new one to LillyDirect.
  • Watch for a text or email. A partner pharmacy contacts you within a few business days to finish the order.
  • Choose self-pay and add needles. Pick pen needles for the KwikPen or needles and syringes for the vial. Most credit cards and HSA or FSA cards work.
  • Choose delivery or pickup. Home delivery is free, and so is Walmart Pharmacy pickup.
  • Set a refill reminder. The pharmacy sends reminders, but from 7.5 mg up the 45-day clock is yours to watch.

The pharmacy cannot change your medicine or your dose. Moving up a step needs a new prescription from your prescriber.

Still comparing cash routes before you order? The cheapest ways to get Zepbound ranks them side by side.

No prescription yet? Who can write it for LillyDirect

LillyDirect sells the medicine but does not prescribe it. You have three simple doors.

  • Your own doctor or gynecologist. If a gynecologist already manages your hormone therapy, they can write it and send it to LillyDirect. Your records stay in one place.
  • A telehealth service Lilly lists. LillyDirect shows a short list of independent telehealth services, including WeightWatchers. Each clinician uses their own judgment, and Lilly does not endorse them.
  • A telehealth company that sells Zepbound at Lilly's price. Ro and Joi both prescribe it, and the drug stays at Lilly's self-pay prices. Their own fee comes on top.

If you take a birth control pill, one label detail matters. Zepbound's label advises switching to a non-oral method, or adding a barrier method, for 4 weeks after starting and after each dose increase.

Our guide to getting a tirzepatide prescription compares the ways to get one and what each costs.

No prescriber yet? Ro can write it
Ro prescribes Zepbound at Lilly's prices, with a required membership of about $150 a month. The first month is $39, refunded if you are not eligible.
See Ro's Zepbound plan

The 45-day refill rule, and what a late refill costs

The rule covers only 7.5 mg, 10 mg, 12.5 mg and 15 mg. At 2.5 mg and 5 mg, there is no clock.

To keep the $449 price, finish your next purchase within 45 days of your last fill. Lilly counts from the day the box was delivered or picked up, not the day you ordered.

  • Late at 7.5 mg: $499 for that fill
  • Late at 10 mg, 12.5 mg or 15 mg: $699 for that fill

A late fill at 10 mg or above costs about $250 more. Reordering around day 30 after each delivery leaves room for a slow shipment.

Think you paid more than the offer allows? Lilly takes reimbursement claims through its patient portal until March 31, 2027. If LillyDirect stops fitting your budget, Zepbound without insurance covers the other cash routes.

Zepbound vials or KwikPen on LillyDirect?

Both cost the same at each dose, so this is a comfort choice.

  • Single-dose vial: a box of four small vials, one per week. You draw each dose with a needle and syringe, which are not included.
  • KwikPen: one prefilled pen that holds four fixed weekly doses. You attach a new pen needle each week. Pen needles differ from vial needles, so order the right kind.

The format also decides where you can buy. Local pharmacies sell the KwikPen at Lilly's self-pay price. Medicare's GLP-1 Bridge covers only the KwikPen. Vials come through LillyDirect, by delivery or Walmart pickup.

For the dose steps and how they are spaced, see our Zepbound dosage guide.

Delivery, cold storage and who LillyDirect serves

Home delivery is free. A partner pharmacy confirms your order within a few business days. Lilly does not post a delivery time on its Zepbound page, so leave some slack before day 45.

Zepbound belongs in the fridge, at 36°F to 46°F. Put the box away as soon as it arrives.

The label allows short stretches outside. Single-dose vials can sit at up to 86°F for 21 days in total. An unopened KwikPen can stay at room temperature for up to 30 days.

Lilly's self-pay offer is for adults 18 and older living in the US or Puerto Rico. Would you rather skip the mailbox? Walmart Pharmacy pickup through LillyDirect is free.

Insurance, the Savings Card, Medicare and HSA or FSA

LillyDirect accepts most major insurance plans for select medicines. The pharmacy checks your coverage and whether your plan needs prior authorization, an approval step before it pays.

If your plan covers Zepbound

The Zepbound Savings Card can bring a fill to as little as $25. That offer names the single-dose pen. The card pays up to $100 a month and $1,300 a year, and it ends December 31, 2026.

If your plan excludes Zepbound, the card on the KwikPen gives you the same self-pay prices as LillyDirect. People on Medicare, Medicaid, TRICARE, VA or other government plans cannot use it. Our Zepbound savings guide explains activation, and Zepbound insurance covers approvals and appeals.

On Medicare: the $50 GLP-1 Bridge

Since July 1, 2026, eligible Part D members pay $50 for a one-month supply of the Zepbound KwikPen. The program runs through December 31, 2027. Vials and single-dose pens are not covered.

  • BMI of 35 or more: no other condition needed.
  • BMI of 30 or more: plus heart failure with preserved ejection fraction, uncontrolled high blood pressure, or chronic kidney disease at stage 3a or higher.
  • BMI of 27 or more: plus prediabetes, a past heart attack or stroke, or blocked leg or arm arteries that cause symptoms.
  • Left out: people with type 2 diabetes, moderate to severe sleep apnea or fatty liver disease, and anyone whose Part D plan already covers a GLP-1.

Your prescriber files a prior authorization and confirms you follow a diet and exercise program. Extra Help does not lower the $50. The Bridge also covers Wegovy and Foundayo.

Paying with an HSA or FSA

LillyDirect takes HSA and FSA cards. Lilly notes that eligibility varies by plan, so check with your benefits administrator. Self-pay purchases do not count toward an insurance deductible.

LillyDirect vs Ro, Joi, LifeMD, PlushCare and your local pharmacy

None of these change Lilly's drug price. What changes is the fee on top, and what that fee buys you.

Ro: a prescription, coaching and insurance help

Ro prescribes Zepbound and charges Lilly's self-pay prices for the drug. A Ro Body membership is required on top.

  • Membership: about $150 a month. The first month is $39, refunded if you are not eligible. Prepaying a year brings it to about $75 a month.
  • All-in on the monthly plan: about $450 to $600 a month, by dose.
  • Included: clinician visits, coaching, insurance help and a metabolic lab test when your provider orders one.

Ro Body is not offered in Hawaii, Louisiana, Mississippi or Virginia. Trustpilot reviewers give Ro 3.9 out of 5. Read our Ro review and Ro cost breakdown.

Joi: weight care and hormone therapy with one team

Joi is a hormone-first telehealth company. Its clinicians prescribe estradiol and progesterone, and it also runs a Zepbound Weight Loss Program.

  • Program fee: about $100 a month, billed about $300 every 3 months.
  • The drug: paid to LillyDirect at Lilly's self-pay price, so about $400 to $550 a month all-in.
  • Included: virtual visits, health coaching and lab access.

Joi fits a woman in her 40s or 50s who wants weight and menopause care from one clinician. The catch is billing. You pay three months at a time, and Joi's terms say products are non-refundable.

If a Joi provider decides Zepbound is not right for you, you are refunded. Trustpilot reviewers give Joi 4.6 out of 5.

Weight care and HRT with one team
Joi's Zepbound program costs about $100 a month, billed every 3 months, plus Lilly's price for the drug.
See Joi's Zepbound program

LifeMD: the same model as Ro, plus a benefits check

  • Membership: about $150 a month after a $39 first month. You can cancel the monthly plan anytime.
  • Yearly plan: about $75 a month, paid upfront.
  • The drug: billed separately at Lilly's self-pay price, or your copay if your plan covers it.

LifeMD is a publicly traded telehealth company that checks your insurance benefits. Membership fees already charged are not refunded. Trustpilot reviewers give it 3.9 out of 5. Our LifeMD cost breakdown has the details.

PlushCare: a doctor visit, no required membership

PlushCare is a primary care telehealth practice. Its doctors can prescribe Zepbound, and the drug stays at Lilly's price.

  • With insurance: PlushCare is in-network with major carriers, so you pay your visit copay, often $30 or less.
  • Without insurance: about $130 for the first visit and about $100 per follow-up.
  • Membership: optional, about $20 a month.

Its doctors also prescribe hormone therapy as pills, patches, gels, creams or rings. Labs are needed at intake and again at six months, billed separately. Trustpilot reviewers give PlushCare 3.4 out of 5. See our PlushCare cost breakdown.

Your local pharmacy: the KwikPen at the same price

Since March 16, 2026, major pharmacies nationwide sell the Zepbound KwikPen at Lilly's self-pay prices. Bring a KwikPen prescription and ask for the self-pay price.

Watch the format at the counter. A GoodRx coupon on the older single-dose pen still runs about $995 to $1,050 a month. That is more than double the KwikPen price, so treat it as a last resort.

Other doors: WeightWatchers, Hims & Hers and Push Health

  • WeightWatchers Clinic: on Lilly's own telehealth list. After a $25 first month, membership is about $75 a month on a 12-month plan or about $85 on a 6-month plan. Zepbound is billed at Lilly's prices. See our WeightWatchers review.
  • Hims & Hers: sells Zepbound at Lilly's prices, with a membership of about $150 a month after a $39 first month. In July 2026, the FTC, Utah and California sued the company over hard-to-cancel subscriptions and health data sharing. Hims denies the claims. See our Hims review.
  • Push Health: a prescription per request, about $65 to $200, with no membership. You then buy Zepbound at Lilly's price. See our Push Health cost guide.

What a year on LillyDirect costs

Zepbound comes in 28-day boxes, so a year means 13 fills. These totals use today's prices and assume every refill lands within 45 days.

  • Following the label's dose steps: $299, then $399, then 11 fills at $449. That comes to about $5,650.
  • Low end, staying at 5 mg all year: $299, then 12 fills at $399. That comes to about $5,100.
  • One late refill at 10 mg or above: adds about $250.

A telehealth service adds its own fee to the label-steps total.

  • Joi: about $1,200 a year in program fees, so about $6,800.
  • Ro or LifeMD on the monthly plan: about $1,680 in the first year, so about $7,300.
  • Ro prepaid for a year: about $890 in fees, so about $6,500.
  • LifeMD yearly plan: about $900 in fees, so about $6,550.
  • PlushCare: visit fees only, or copays if your plan covers visits.

Lilly's current self-pay card terms run through December 31, 2026. Lilly can change the offer at any time.

Is LillyDirect cheaper than compounded tirzepatide?

Usually not, but compare like with like.

  • Drug only at LillyDirect: $299 at 2.5 mg, $399 at 5 mg and $449 from 7.5 mg up. You still need a prescriber on top.
  • All-in at one company: Joi charges about $250 a month for compounded tirzepatide, visits included, billed every 3 months. Its Zepbound program runs about $400 to $550 a month with Lilly's drug price.

Compounded tirzepatide is not Zepbound. A pharmacy mixes it for one patient, and the FDA has not approved it as a finished drug. Since the shortage ended in December 2024, a prescriber needs a documented medical reason beyond price.

On April 30, 2026, the FDA proposed not to add tirzepatide to the 503B bulks list. That list governs large outsourcing pharmacies. The comment period has closed, and no final decision has been published.

Our compounded tirzepatide comparison lists sellers, prices and pharmacy types.

What else LillyDirect sells: Mounjaro and Foundayo

  • Mounjaro: the same molecule, approved for type 2 diabetes. Self-pay is about $500 a month at every dose. See Mounjaro cost.
  • Foundayo: Lilly's daily weight-loss pill. Self-pay runs from $149 to $299 a month by dose, with two promo prices through December 31, 2026. The top doses need a refill within 45 days to stay at $299. See Foundayo cost.

If your prescription is for weight management, Zepbound's prices apply. A Mounjaro offer does not carry over to Zepbound.

Red flags: fake LillyDirect sites and messages

A famous name draws copycats. The real service lives at lilly.com/lillydirect. Its partner pharmacies are Amazon Pharmacy, Fuze Health, Gifthealth and Walmart Pharmacy.

  • No prescription asked: LillyDirect never ships Zepbound without one. A site that skips that step is not Lilly.
  • Brand Zepbound for cash well under $299: that is Lilly's own starting price. Only insurance and Medicare go lower.
  • A web address that is not lilly.com: check it before you enter a card number.
  • An unexpected text or email about an order: if you are unsure it is real, call LillyDirect customer care at 1-844-559-3471 before replying.

The FDA's advice points the same way. Use a licensed prescriber and a state-licensed pharmacy, and be wary of unusually low prices.

Frequently asked questions

How much is Zepbound on LillyDirect?+

It is $299 a month at 2.5 mg and $399 at 5 mg. From 7.5 mg to 15 mg it is $449, if you refill within 45 days. The vial and the KwikPen cost the same. Each fill is a 28-day supply, and needles are extra.

Does LillyDirect prescribe Zepbound?+

No. LillyDirect is a pharmacy service, not a medical practice. Your own clinician can send the prescription, or you can use a telehealth service. LillyDirect lists several independent ones, and Ro and Joi also prescribe it.

Is the Zepbound vial cheaper than the KwikPen on LillyDirect?+

No. Both follow the same price at each dose. The vial needs a needle and syringe for each weekly dose. The KwikPen holds four weekly doses and takes pen needles.

What happens if I miss the 45-day refill window?+

At 7.5 mg, that fill costs $499 instead of $449. At 10 mg, 12.5 mg or 15 mg, it costs $699. The 45 days run from your last delivery or pickup. The rule does not apply at 2.5 mg or 5 mg.

Can I use LillyDirect if I have Medicare?+

Yes. Lilly says self-pay is open to everyone, whatever their insurance. If you qualify for Medicare's GLP-1 Bridge, the Zepbound KwikPen costs $50 a month through December 31, 2027. Lilly's Savings Card does not work with Medicare or other government plans.

Can I pay for LillyDirect Zepbound with an HSA or FSA?+

Yes, LillyDirect takes HSA and FSA cards. Eligibility depends on your plan, so check with your benefits administrator.

Can I get the LillyDirect price at my local pharmacy?+

For the KwikPen, yes. Since March 16, 2026, major pharmacies nationwide sell it at Lilly's self-pay prices. Vials come through LillyDirect, by home delivery or free Walmart Pharmacy pickup.

How long does LillyDirect take to ship Zepbound?+

Lilly does not post a delivery time. A partner pharmacy contacts you within a few business days after your prescription arrives. Delivery is free, so order refills early to stay inside the 45-day window.

Is LillyDirect cheaper than compounded tirzepatide?+

Not usually. At Joi, compounded tirzepatide costs about $250 a month with visits included, against about $400 to $550 for its Zepbound program. But compounded tirzepatide is mixed by a pharmacy and is not FDA-approved. LillyDirect sells the approved brand from the maker.
