HRT Online in Iowa: 2026 Prices, Providers and What Wellmark, Iowa Medicaid or Medicare Will Pay

Iowa lets nurse practitioners prescribe on their own, and a third of its 99 counties have no OB-GYN. For many women from Sioux City to Dubuque, an online clinician is the quickest way to start hormone therapy.

The short answer

Yes, an Iowa-licensed clinician can prescribe hormone replacement therapy, or HRT, online with no office visit first, but Iowa's medical board bans prescribing from a fixed online questionnaire alone. Estrogen plus progesterone from a service that ships your medicine costs about $65 to $200 a month. Midi Health says it's in-network with most PPO plans, but none of these online services takes Iowa Medicaid.

Updated October 26, 2026. That day we read the Iowa Board of Medicine's telemedicine rule, 481-655.9, and the nurse practitioner rules in chapter 481-621. We also read Iowa Code section 514C.34, Iowa HHS pages on the Iowa Health and Wellness Plan, and Wellmark's Blue Rx Value Plus drug list. Service prices come from each provider's own site, checked in October 2026.

Match your situation to one line below, then find that provider in the table.

  • Cheapest estrogen and progesterone prescribed and shipped to you: Alloy, about $65 a month for an estradiol pill plus progesterone, billed every three months, after a one-time $49 consult.
  • Pay per product with no consult fee: Winona, about $90 a month for its combined estrogen and progesterone cream, or about $95 for tablets plus a progesterone capsule.
  • A fee that halves: Inner Balance, about $100 a month from the seventh month.
  • A PPO plan: Midi Health, which says it's in-network with most PPO plans.
  • A low-cost membership with messaging: Evernow, about $35 a month on a prepaid year, with medicine extra.
  • You already have a prescriber: coupon generics, or The HRT Club.
  • On Iowa Medicaid: an Iowa clinician who takes your IA Health Link plan, since none of these online services does.

What does HRT cost in Iowa, and which online services prescribe here?

Every service below charges one national price, so a woman in Decorah pays what one in Des Moines pays. Iowa decides how the first visit must work, who may prescribe, and what your coverage pays.

There are three ways to pay. Some services fold the clinician and the medicine into one bill. Others charge for care alone, through a membership or your insurance, and send the prescription to a pharmacy. A generic filled with a free coupon card costs least, once someone has written the prescription.

One name you may know is missing. Fountain doesn't list Iowa among the states it serves, so it isn't in the table.

Winona: best for paying per product with no consult fee

Winona lists Iowa among its 37 states, plus Puerto Rico. There's no consult fee and no bloodwork, and you pay for each product. Progesterone capsules alone cost about $40 a month, and estrogen tablets about $55.

For both hormones, the combined estrogen and progesterone cream costs about $90 a month. Tablets with a progesterone capsule come to about $95. The estradiol patch costs the most, at about $150 before progesterone.

The patch, tablets and capsules are FDA-approved. The creams are compounded, meaning mixed to order, in two pharmacies Winona owns. Its terms also let you ask the doctor to send a prescription to a pharmacy you choose.

Care runs by written intake and doctor messaging, with no video visit and no testosterone. Winona holds 4.6 on Trustpilot from about 8,600 reviews, and complaints center on automatic refills and cancelling. Winona cost prices every product.

Want HRT in Iowa without a video call?
Winona serves Iowa with no consult fee and no labs. Its estradiol patch, tablets and progesterone capsules are FDA-approved, and HSA or FSA cards work.
See Winona's Iowa pricing

Inner Balance: best for one cream whose price halves after six months

Inner Balance charges about $200 a month for the first six months. That covers the evaluation and Oestra, its only treatment. Oestra is one compounded cream that holds estradiol and progesterone. You apply it to the vaginal tissue, and it acts throughout the body.

From month seven, the fee falls to about $100. The cream ships every three months and is billed monthly. Labs are optional, and a note on its pricing page says a dose change may cost extra.

The practice behind it, Inner Balance Health PLLC, is based in Boise, Idaho. It holds 2.2 on Trustpilot from 38 reviews, and most complaints involve cancelling. Its refund guarantee has two deadlines, which our review lays out.

Prefer one cream to a patch and a pill?
Inner Balance costs about $200 a month for six months, then about $100. Its Oestra cream is compounded, labs are optional, and HSA or FSA cards work.
See Inner Balance pricing

Alloy: best for the cheapest estrogen and progesterone prescribed and shipped

Alloy treats menopause in all 50 states and DC. You pay a one-time $49 consult, then a price for each product, with no membership. Alloy says its consult doctors are certified by The Menopause Society, the main US medical society for menopause care.

The estradiol pill costs about $40 a month, and progesterone starts near $25. No service in this table that both prescribes and ships your medicine charges less for that pair. The estradiol patch costs about $100 a month.

Everything ships as a three-month supply, billed once a quarter. For the pill with progesterone, each charge comes to about $190. Code PILL25 takes 25 percent off a first order of the pill.

Care runs by message, with no video and no required labs. Alloy holds 4.3 on Trustpilot from about 4,040 reviews. Alloy cost prices every product.

Midi Health: best if your Iowa plan is a PPO

A PPO, or preferred provider organization, is a plan that lets you see specialists without a referral. Midi Health says it's in-network with most PPO plans. In network, you pay your specialist copay, a set fee per visit, plus any deductible you still owe.

Midi names Blue Cross Blue Shield among the carriers it works with, but not Wellmark by name. Wellmark is Iowa's Blue Cross Blue Shield company, so check your plan in Midi's coverage tool before you book.

Without coverage, the first visit costs $250 and each follow-up costs $150. Medicare is out of network, and Midi doesn't accept Medicaid. Visits happen by video, and labs and medicine are billed on their own.

Midi holds 4.1 on Trustpilot from about 1,700 reviews. Midi Health and insurance goes plan by plan, and Winona vs Midi Health weighs cash against insurance.

Have a PPO plan in Iowa?
Midi says it's in-network with most PPO plans, so a menopause visit often costs your specialist copay. Without coverage, the first visit is $250 and follow-ups are $150.

Evernow: best for messaging a clinician between visits

Evernow centers on menopause care and says it serves all 50 states plus DC. Membership costs $49 month to month, or $129 for three months. A year paid upfront costs $420, which works out to about $35 a month.

Membership covers round-the-clock messaging with a clinician, prescriptions and refills. Video visits cost extra. Evernow bills Aetna, Anthem, Blue Cross Blue Shield and UnitedHealthcare plans for them, or charges $150 without coverage. Medicine is billed separately.

Its estradiol patches and vaginal estradiol cream are FDA-approved. The cream comes free on the 3- or 12-month plan if you qualify. Some items, such as a facial estriol cream, are compounded.

Its Trustpilot profile is tiny and unclaimed, at 2.0 from 15 reviews, too few to judge either way.

The HRT Club: best if a clinician already writes your prescription

The HRT Club is not a clinic. It's a mail-order pharmacy membership with a free directory of prescribers. The basic tier costs nothing, and membership costs $12 a month or $99 a year.

Members pay $15 for 30 capsules of generic progesterone, against $135 for guests. Shipping costs $10 an order, and annual members ship free on orders of $50 or more.

It suits you if an Iowa clinician already prescribes for you and you pay cash. The catches are no refunds, an annual plan you can't end early, and no Trustpilot reviews so far.

Generic estradiol and progesterone at an Iowa pharmacy

With a prescription in hand, a generic plus a free coupon card usually costs least. The coupon row uses national prices, not a quote for a Des Moines, Cedar Rapids or Sioux City ZIP code.

GoodRx lists a month of generic estradiol patches, eight twice-weekly patches, at about $30. Thirty capsules of micronized progesterone, the body-identical form of the hormone, run about $15 with the coupon. On SingleCare, a second free card, 30 estradiol tablets cost about $5.

Bigger fills can cost less per month. On October 26, SingleCare showed 90 progesterone capsules for about $35 at its lowest-priced pharmacies.

Online services leave this door open. Winona's and Alloy's terms let you send your prescription to a pharmacy you pick. With insurance, compare the coupon price with your copay and pay whichever is lower.

Our HRT savings guide shows how the discount cards work. For every low-cost route side by side, see the cheapest HRT options.

  • Inner Balance: about $1,790 in the first year, then about $1,195 a year.
  • Alloy, pill and progesterone: about $805 in year one with the consult, then about $755, paid in four quarterly charges. The patch with progesterone runs about $1,525 in year one, then about $1,475.
  • Evernow: about $570 for a prepaid year plus one self-pay visit, or about $740 paying monthly. Medicine is extra.
  • Midi Health: your specialist copay for each in-network visit. Paying cash, a first visit and three follow-ups come to $700, before medicine.
  • Your own prescriber with coupon generics: about $240 a year for tablets and progesterone. With patches, it's about $570, counting 13 four-week patch boxes. Add visit fees, plus $99 a year if you fill through The HRT Club.

Yes. The Iowa Board of Medicine's telemedicine rule, 481-655.9, lets an Iowa-licensed physician start a patient relationship by telemedicine. That holds whenever the standard of care doesn't call for an in-person visit.

The rule took its current number in 2025, when Iowa moved its licensing boards under the Department of Inspections, Appeals and Licensing. It used to sit in chapter 13 of the medical board's rules.

Iowa's definition covers live video. It also covers asynchronous store-and-forward care, meaning health information sent for a clinician to review later. Care given only by audio-only phone, email or fax doesn't count as telemedicine.

A fixed online questionnaire isn't enough in Iowa

Before prescribing, the physician must take your medical history and do any exam that's medically necessary. The rule says a static questionnaire, with set questions and set answers, isn't an acceptable medical interview.

Prescribing based solely on an internet questionnaire is banned outright. The rule contrasts that with an adaptive, interactive online interview, where the clinician's questions respond to your answers.

The rule lists other duties too. The physician must confirm who you are and let you check their license. They must know the follow-up care near you and, when appropriate, send your record to your regular doctor.

So expect more than a form. Winona, Alloy and Inner Balance start with a written health history, then a clinician reviews it and follows up by secure message.

Midi holds its visits by video, and Evernow adds paid video visits to its messaging. If you'd rather talk face to face first, choose a video service.

Your clinician needs an Iowa license

What counts is where you are during the visit. A physician treating you in Iowa needs an active Iowa medical license, even when the company sits in another state. A nurse practitioner needs an Iowa license from the Iowa Board of Nursing.

Iowa enacted the Interstate Medical Licensure Compact in 2015, the tenth state to do so. It gives doctors licensed in other member states a faster way to add an Iowa license.

Iowa is also in the Nurse Licensure Compact. That compact covers registered nurses, not nurse practitioners.

Iowa nurse practitioners prescribe on their own

In Iowa, a nurse practitioner is licensed as an advanced registered nurse practitioner, or ARNP. Under rule 481-621.4, an ARNP may prescribe drugs, including controlled substances, within their certified area of practice.

Iowa's licensing department says ARNPs may practice independently. A collaborative agreement with a physician is optional, and the Board of Nursing doesn't require one.

The ARNP telehealth rule, 481-621.9, lets the relationship start by telehealth, held to the same standard of care as an office visit. So an Iowa ARNP can evaluate you online and prescribe estradiol and progesterone without a supervising doctor.

Testosterone has extra rules

Testosterone is a Schedule III drug, a federal class of controlled medicines with stricter handling. The prescriber needs a DEA registration. Iowa adds its own controlled substances registration, issued through the Iowa Board of Pharmacy.

For a first prescription without an in-person exam, prescribers rely on federal telemedicine flexibilities. They run through December 31, 2026. The DEA's permanent rule on a special telemedicine registration is under White House review, and its terms aren't public yet.

Iowa's rule to check the Prescription Monitoring Program before prescribing applies to opioids. The program still tracks Schedule II through IV drugs, so a testosterone fill shows up there.

Midi lists a testosterone cream in its online store, from $100 per 90 days. Winona and Evernow don't list testosterone. No testosterone product is FDA-approved for women, and the 2019 global consensus statement supports it only for low sexual desire after menopause.

Will Wellmark, Iowa Medicaid or Medicare pay for hormone replacement therapy in Iowa?

Coverage usually reaches the hormones themselves, but never a telehealth subscription. Visits fall in between, because Midi and Evernow bill some plans for them.

Iowa's telehealth coverage law, Iowa Code section 514C.34, bars state-regulated plans from covering a service less generously because it's delivered by telehealth. It reaches individual and group health insurance, HMOs and the state's public employee plans.

Under that law, telehealth means real-time audio and video. Care given only by audio-only phone, email or fax falls outside it. A message a clinician answers later isn't real-time either.

The law requires equal pay rates for telehealth only in mental health care. For other visits, your plan sets the rate.

These rules don't reach every plan. Self-funded employer plans, common at large companies, follow a federal law called ERISA instead.

Winona, Alloy and Inner Balance don't bill any insurer, and no plan covers Evernow's membership. HSA and FSA cards, the tax-free health accounts many employers offer, work at all four. Winona and insurance covers that route for a Winona order.

Our HRT insurance guide explains how to read a drug list and appeal a denial.

Wellmark Blue Cross and Blue Shield of Iowa

Every plan publishes a formulary, its list of covered drugs sorted into price tiers. The lower the tier, the less you pay.

Wellmark's Blue Rx Value Plus drug list, in its September 2025 version, puts generic estradiol tablets, weekly patches and gel on tier 1. That's the lowest-cost tier.

Twice-weekly estradiol patches are tier 1 too, with a quantity limit of eight patches per 28 days. A quantity limit caps how much you can fill at once. Progesterone capsules are tier 1, with no limit listed.

Testosterone gel sits on tier 2 and needs prior authorization, meaning approval from the plan before you fill. Testosterone cypionate injections are tier 1.

Plans built on this list can merge tiers, so a two-tier or one-tier plan prices them differently. Your plan may use another Wellmark list, so check yours in your member account or call the number on your card.

For visits, Midi and Evernow both name Blue Cross Blue Shield among the plans they bill. Neither names Wellmark, so confirm your network status before you book.

HealthCare.gov in Iowa

Iowa has no state-run marketplace, so you buy an individual plan on HealthCare.gov. Six insurers sold plans there for 2026: Wellmark, Medica, Oscar, UnitedHealthcare, Iowa Total Care's Ambetter and Avera, which was new that year.

Medica leaves Iowa's individual market at the end of 2026. The other five filed 2027 rates with the Iowa Insurance Division. If you buy your own plan, compare each drug list for estradiol and progesterone before you switch.

Iowa Medicaid

Iowa expanded Medicaid through the Iowa Health and Wellness Plan, for adults 19 to 64. Since April 1, 2026, the state lists the limit as 133 percent of the federal poverty level.

That's about $21,227 a year for one person, or roughly $1,770 a month. When your income is counted, a federal rule takes off 5 percent of the poverty level. So in practice you can qualify up to about 138 percent, roughly $1,835 a month.

From the second year, some members owe a small monthly contribution, depending on income. Most members get care through IA Health Link, the state's managed care program. Its three health plans are Iowa Total Care, Molina Healthcare and Wellpoint.

Drug coverage follows the state's preferred drug list, which the health plans use too. A drug marked non-preferred needs prior authorization before you fill it.

Iowa Medicaid pays for appropriate telehealth as it pays for in-person care, with no office visit required first.

A work rule is starting. From December 1, 2026, new applicants must show 80 hours a month of work, school, job training or community service, or $580 a month in earnings.

Current members face the rule at renewals from February 2027. Exemptions include people who are medically frail and caregivers of a child under 14, among others.

None of the online services above takes Iowa Medicaid. An Iowa clinician who accepts your IA Health Link plan is the realistic route.

Medicare

Most Part D drug plans list generic estradiol and progesterone. Your 2026 out-of-pocket spending on covered drugs stops at $2,100.

Midi treats Medicare as out of network, so visits cost its self-pay rate. Evernow doesn't accept Medicare.

Finding a menopause doctor or menopause specialist in Iowa

The Menopause Society's practitioner directory, at portal.menopause.org, is the usual first stop. It lists members and Menopause Society Certified Practitioners, known as MSCPs, and you search it by ZIP code.

University of Iowa Health Care runs a Menopause Health Clinic in Iowa City, inside its obstetrics and gynecology department. It's led by Holly-Marie Bolger, DO, a clinical associate professor of obstetrics and gynecology.

In Des Moines, Cedar Rapids or Davenport, run the directory with your own ZIP code. It's the surest way to find a clinician who has trained in menopause care.

Distance is the hard part. A 2023 March of Dimes report found 33 of Iowa's 99 counties had no OB-GYN and no birthing hospital or center.

The AMA counts 3.3 OB-GYNs per 10,000 women of reproductive age in Iowa, against 4.5 nationally. That figure was reported in March 2025.

If the nearest specialist means a long drive or a long wait, online care can be the faster start. A local visit still adds what a screen can't, such as a hands-on pelvic exam.

To size up a local clinician, see our guide to finding perimenopause care. HRT near me helps you weigh an online clinician against the closest office, and finding an HRT doctor covers what to ask.

Bioidentical hormones, compounded creams and pellets in Iowa

Bioidentical describes the shape of a hormone, not how it's made. The molecule matches the estradiol or progesterone your ovaries produce. Most estradiol patches and micronized progesterone capsules meet that test, and they're FDA-approved too.

Compounded hormones are mixed by a pharmacy for one patient and never go through FDA approval. In Iowa, the Board of Pharmacy, now part of the licensing department, oversees pharmacies.

A pharmacy in another state must hold an Iowa nonresident pharmacy license before it ships to Iowa residents. For compounding, the board points to USP chapters 795 and 797, the national standards for nonsterile and sterile preparations.

Pellets are small compounded hormone implants placed under the skin every few months. Biote's provider directory has Iowa city pages for Cedar Rapids, West Des Moines, Ankeny, Urbandale, Davenport and Council Bluffs, among others.

We found no Iowa clinic that posts its pellet prices online. Expect to pay cash, and ask for a written quote that covers the pellets, insertion and labs. Read our hormone pellets guide before you book.

At the FDA's request, the National Academies reviewed compounded bioidentical hormones in 2020. They found little evidence of safety or benefit. ACOG and The Menopause Society both prefer an FDA-approved product whenever one exists.

Approved products are getting new safety labels. On February 12, 2026, the FDA approved revised labels for a first group of six hormone products. Statements on heart disease, breast cancer and probable dementia came out of the boxed warning, a label's most prominent caution.

That first group included Prometrium, a brand of micronized progesterone, and Divigel, an estradiol gel. No estradiol patch was in it.

In the table, Winona is mixed. Its patch, tablets and capsules are FDA-approved, while its creams are compounded.

Alloy's hormone protocol, Midi's default treatments, Evernow's patches and vaginal cream, The HRT Club's drugs and the coupon generics are FDA-approved. Inner Balance's Oestra is compounded, a systemic cream applied vaginally. If you want compounded hormones on purpose, compounded HRT: where to get it covers what to check.

Starting HRT in your 40s in Iowa: labs, age and the questions to ask

Being told you're too young for hormones is common in your 40s. The Menopause Society's 2022 position statement says benefits outweigh risks for most healthy women with symptoms. That holds under age 60, or within ten years of menopause.

Perimenopause comes before the last period, so it falls inside that window. Hormone levels swing too much in these years for a blood test to confirm it. Winona and Alloy both prescribe without required labs.

Wondering whether to test anyway? Perimenopause labs covers what a hormone panel can tell you, and what it can't.

Before you commit, put these four questions to any Iowa prescriber, on a screen or in an exam room.

  • Training: Have you trained in menopause care, or earned The Menopause Society's certification?
  • First choice: Do you begin with FDA-approved estradiol and micronized progesterone, and turn to compounded hormones only if those don't suit me?
  • License: Do you hold an Iowa license? If you're an ARNP, which national certification do you hold?
  • Format: Beyond my written history, how will you assess me: an interactive online interview, a phone call or a video visit?

Once you start, our HRT dosage guide walks through how the dose gets adjusted.

Frequently asked questions

Is it legal to get HRT online in Iowa without an in-person visit?+

Yes. Under the Iowa Board of Medicine's rule 481-655.9, an Iowa-licensed physician may start a patient relationship by telemedicine when the standard of care doesn't require an in-person visit. Live video and asynchronous store-and-forward care both count. A static online questionnaire with set answers does not, and prescribing based solely on one is banned. Expect an interactive exchange with the clinician, by message or video, before a prescription.

Can an Iowa nurse practitioner prescribe HRT without a doctor?+

Yes. Under rule 481-621.4, an Iowa ARNP may prescribe drugs, including controlled substances, within their certified area of practice. Iowa's licensing department says ARNPs may practice independently, and a collaborative agreement with a physician is optional. The ARNP telehealth rule lets the relationship start online. For testosterone, the ARNP also needs a DEA registration and an Iowa controlled substances registration.

Does Iowa Medicaid cover HRT, and who qualifies in 2026?+

Iowa covers adults 19 to 64 through the Iowa Health and Wellness Plan. Since April 1, 2026, the state lists the limit as 133 percent of the federal poverty level, about $21,227 a year for one person. Because a federal rule takes 5 percent of the poverty level off counted income, you can qualify up to about 138 percent, roughly $1,835 a month. Members get care through Iowa Total Care, Molina or Wellpoint. From December 1, 2026, new applicants must show 80 hours a month of work or other activities, or $580 in monthly earnings. None of the online services on this page accepts Iowa Medicaid.

Does Wellmark cover estradiol, progesterone and telehealth visits?+

Wellmark's Blue Rx Value Plus drug list, in its September 2025 version, puts generic estradiol tablets, patches and gel on tier 1, the lowest-cost tier. Progesterone capsules are tier 1 too. Twice-weekly patches carry a limit of eight per 28 days, and testosterone gel is tier 2 with prior authorization. Iowa Code section 514C.34 bars state-regulated plans from covering a live video visit less generously than an office visit. Self-funded employer plans follow ERISA instead, and no plan pays a subscription fee.

Can I get testosterone for menopause online in Iowa?+

Possibly, with conditions. Testosterone is Schedule III, so the prescriber needs a DEA registration and an Iowa controlled substances registration. Starting it without an in-person exam relies on federal telemedicine flexibilities that run through December 31, 2026. Midi sells a testosterone cream in its store, from $100 per 90 days. No testosterone product is FDA-approved for women, and the evidence supports it only for low sexual desire after menopause.

Where can I see a menopause specialist in person in Des Moines, Cedar Rapids or Iowa City?+

In Iowa City, University of Iowa Health Care runs a Menopause Health Clinic, led by Holly-Marie Bolger, DO. For Des Moines, Cedar Rapids or anywhere else, search The Menopause Society's directory at portal.menopause.org by ZIP code. It lists members and certified practitioners. A 2023 March of Dimes report found 33 of Iowa's 99 counties had no OB-GYN, so online care often comes first in rural Iowa.
