HRT Online in Hawaii: 2026 Prices, Providers and What HMSA, Kaiser, Med-QUEST or Medicare Will Pay

A Hilo woman who would rather not wait for a gynecology opening can start hormone therapy from her lanai, as long as her clinician holds a Hawaii license. Her medicine then arrives by mail, or waits for her at a pharmacy down the road.

The short answer

Yes, Hawaii law lets a Hawaii-licensed clinician start hormone replacement therapy, or HRT, online, but a prescription based only on an online questionnaire doesn't count as care. Estrogen plus progesterone prescribed and shipped costs about $65 to $200 a month. HMSA's Essential drug list for commercial plans puts generic estradiol and progesterone on its lowest tier, but none of these online services takes Med-QUEST.

Fountain. Coverage as published by each brand on Oct 1, 2026; it changes often.

Updated November 3, 2026. That day we read Hawaii's physician telehealth law, section 453-1.3 of the Hawaii Revised Statutes. We also read HMSA's commercial and QUEST drug lists, the Med-QUEST plan page and the state's latest physician workforce report. Service prices come from each provider's own site, checked in October 2026.

Find the line below that fits your situation, then look up that provider in the table.

  • Pay per product with no consult fee: Winona, about $90 a month for its combined estrogen and progesterone cream.
  • One cream whose price halves: Inner Balance, about $200 a month for six months, then about $100.
  • Cheapest estrogen and progesterone prescribed and shipped to you: Alloy, about $65 a month for an estradiol pill plus progesterone, after a one-time $49 consult.
  • A PPO plan: Midi Health, which says it's in-network with most PPO plans. Its coverage tool shows where your plan stands.
  • A low-cost membership with messaging: Evernow, about $35 a month on a prepaid year, with medicine extra.
  • You already have a prescriber: coupon generics, or The HRT Club.
  • On Med-QUEST or a Kaiser HMO plan: a clinician inside your own plan. None of these online services takes Med-QUEST, and Kaiser HMO plans generally need a referral for outside care.

Which online HRT services serve Hawaii, and what does HRT cost here?

None of these services charges more for an island address. A woman in Lihue pays the same national price as a woman in Kailua. Hawaii's own rules decide how care may start, who may prescribe, and what your coverage has to do.

You'll meet three ways to pay. Some services bundle the clinician and the medicine into one bill. Others charge only for care, through a membership or your insurance, and send the prescription to a pharmacy. If you already hold a prescription, a generic with a free coupon card is the cheapest route.

One name you may know is missing. Fountain's only published state list leaves out Hawaii, so it isn't in the table.

Winona: best for paying per product with no consult fee

Winona lists Hawaii among its 37 states, plus Puerto Rico. There's no consult fee and no bloodwork. You pay for each product, and progesterone capsules alone cost about $40 a month.

For both hormones, its combined estrogen and progesterone cream costs about $90 a month. Estrogen tablets with a progesterone capsule come to about $95. The estradiol patch costs the most, at about $150 before progesterone.

The patch, tablets and capsules are FDA-approved. The creams are compounded, meaning mixed to order, in two pharmacies Winona owns. Its terms also let you ask the doctor to send a prescription to a pharmacy you choose, such as one near home on Maui or Kauai.

Care runs by written intake and doctor messaging, with no video visit and no testosterone. Winona holds 4.6 on Trustpilot from about 8,600 reviews. Complaints center on automatic refills and cancelling, and Winona cost prices every product.

Want HRT in Hawaii by doctor messaging, no video call?
Winona serves Hawaii with no consult fee and no labs. Its estradiol patch, tablets and progesterone capsules are FDA-approved, and HSA or FSA cards work.

Inner Balance: best for one cream whose price halves after six months

Inner Balance is run by a medical practice in Boise, Idaho, and treats patients online. Its pricing page says its founder is licensed to practice medicine in all 50 states.

It charges about $200 a month for the first six months. That covers the evaluation, clinician check-ins and Oestra, its only treatment. Oestra is one compounded cream that holds estradiol and progesterone. You apply it to the vaginal tissue, and it acts throughout the body.

From month seven, the fee falls to about $100. The cream ships every three months and is billed monthly. Labs are optional, and a note on its pricing page says a dose change may cost extra.

It holds 2.2 on Trustpilot from 38 reviews, and most complaints involve cancelling. Its refund guarantee has two deadlines, which our review lays out. Inner Balance cost breaks down each charge, and our Inner Balance legitimacy check looks at the practice itself.

Prefer one cream to a patch and a pill?
Inner Balance costs about $200 a month for six months, then about $100. Its Oestra cream is compounded, labs are optional, and HSA or FSA cards work.

Alloy: best for the cheapest estrogen and progesterone prescribed and shipped

Alloy treats menopause in all 50 states and DC. You pay a one-time $49 consult, then a price for each product, with no membership. Alloy says its consult doctors are certified by The Menopause Society, the main US medical society for menopause care.

The estradiol pill costs about $40 a month, and progesterone starts near $25. Counting every fee, no other service in this table that both prescribes and ships your medicine costs less for that pair. The estradiol patch costs about $100 a month.

Everything ships as a three-month supply, billed once a quarter. For the pill with progesterone, each charge comes to about $190. Code PILL25 takes 25 percent off a first order of the pill.

Care runs through Alloy's secure portal, with a written intake and then messages with the doctor. Labs aren't required. Alloy holds 4.3 on Trustpilot from about 4,040 reviews, and Alloy cost prices every product.

Want the lowest price for both hormones, shipped?
Alloy serves Hawaii with FDA-approved estradiol and progesterone, a one-time $49 consult and doctors it says are Menopause Society certified.

Midi Health: best if your plan is a PPO

A PPO, or preferred provider organization, is a plan that lets you see specialists without a referral. Midi Health says it's in-network with most PPO plans. In network, you pay your specialist copay, a set fee per visit, plus any deductible you still owe.

Midi says it serves all 50 states, but it has no Hawaii page naming local insurers. Nationally, it names Aetna, Blue Cross Blue Shield, Cigna and UnitedHealthcare. HMSA is Hawaii's Blue Cross Blue Shield company, yet Midi doesn't say HMSA plans are in network. Its coverage tool shows whether yours is.

Without coverage, the first visit costs $250 and each follow-up costs $150. Medicare is out of network, and Midi doesn't accept Medicaid, so Med-QUEST members can't use it. Visits happen by video, and labs and medicine are billed on their own.

Midi holds 4.1 on Trustpilot from about 1,700 reviews. Midi Health and insurance goes plan by plan, and Winona vs Midi Health weighs cash against insurance.

Have a PPO plan in Hawaii?
Midi says it's in-network with most PPO plans, so a menopause visit can cost your specialist copay. Without coverage, the first visit is $250 and follow-ups are $150.

Evernow: best for messaging a clinician between visits

Evernow centers on menopause care and says it serves all 50 states plus DC. Membership costs $49 month to month, or $129 for three months. A year paid upfront costs $420, which works out to about $35 a month.

Membership covers round-the-clock messaging with a clinician, prescriptions and refills. Video visits cost extra. Evernow bills Aetna, Anthem, Blue Cross Blue Shield and UnitedHealthcare plans for them, or charges $150 without coverage. Medicine is billed separately.

Its estradiol patches and vaginal estradiol cream are FDA-approved. On the 3- or 12-month plan, the cream is free if you qualify. Some items, such as a facial estriol cream, are compounded. Evernow doesn't take Medicare or Medicaid.

Its Trustpilot profile is small and unclaimed. It shows 2.0 from 15 reviews, too few to judge either way.

The HRT Club: best if a clinician already writes your prescription

The HRT Club is not a clinic. It's a mail-order pharmacy membership with a free directory of prescribers. The basic tier costs nothing, and membership costs $12 a month or $99 a year.

Members pay $15 for 30 capsules of generic progesterone, against $135 for guests. Shipping costs $10 an order, and annual members ship free on orders of $50 or more.

It suits you if a Hawaii clinician already prescribes for you and you pay cash. The catches are no refunds, an annual plan you can't end early, and no Trustpilot reviews so far.

Generic estradiol and progesterone at a Hawaii pharmacy

If you already have a prescription, a generic plus a free coupon card usually costs least. The coupon row uses national prices, not a quote for a Honolulu, Hilo or Kahului ZIP code.

GoodRx lists a month of generic estradiol patches, eight twice-weekly patches, at about $35. Thirty capsules of micronized progesterone, the body-identical form of the hormone, cost about $15 to $20 with a free coupon card.

SingleCare is another free card. There, 30 generic estradiol tablets cost less than $5.

Patches have been harder to find this fall. The FDA has worked with makers since September to raise supply, without declaring a shortage. Bayer expects one Climara dose to stay tight until early 2027. On an outer island, ask your pharmacy to order each refill a few days early.

Online services leave this door open. Winona's and Alloy's terms let you send your prescription to a pharmacy you pick. With insurance, compare the coupon price with your copay and pay whichever is lower.

Our HRT savings guide shows how the discount cards work. To compare every low-cost route, see the cheapest HRT options.

What a year of HRT costs in Hawaii

  • Winona: about $1,070 a year for the combined cream, or about $1,115 for tablets with a progesterone capsule. The patch with a capsule runs about $2,260.
  • Inner Balance: about $1,790 in year one, then about $1,195 a year.
  • Alloy, pill and progesterone: about $805 in year one with the consult, then about $755, paid in four quarterly charges. The patch with progesterone runs about $1,525 in year one, then about $1,475.
  • Midi Health: your specialist copay for each in-network visit. Paying cash, a first visit and three follow-ups come to $700, before medicine.
  • Evernow: about $570 for a prepaid year plus one self-pay video visit, or about $740 paying monthly. Medicine is extra.
  • Your own prescriber with coupon generics: about $245 to $280 a year for tablets and progesterone. With patches, it's about $645 to $680, counting 13 four-week patch boxes. Add visit fees, plus $99 a year if you fill through The HRT Club.

Yes, it's legal, and the first visit doesn't have to be in person. The rules sit in section 453-1.3 of the Hawaii Revised Statutes, the state's telehealth law for physicians.

Subsection (e) says a physician-patient relationship may be established through a telehealth interaction. The condition is that the physician holds a license to practice medicine in Hawaii.

The law defines telehealth broadly. It covers video, store-and-forward technology, and secure web-based messages exchanged at different times. Store-and-forward means sending a health history or photos for a clinician to review later.

Ordinary phone calls, faxes and plain email don't count as telehealth, whether used alone or together.

A real exchange with a clinician, not a form alone

Subsection (c) draws the line. It says issuing a prescription based solely on an online questionnaire is not treatment and does not meet an acceptable standard of care.

So expect a clinician to read your history and write back with questions or a plan. Winona, Alloy and Inner Balance start with a written health history. A clinician reviews it and follows up by secure message.

Opioids are the one area where the law demands an in-person visit first. Hormone therapy falls outside that rule.

Midi holds its visits by video, and Evernow adds paid video visits to its messaging. If you'd rather talk face to face before a prescription, choose one of those two.

Mind the clock, too. Hawaii doesn't use daylight saving time, so Pacific time on the mainland runs two hours ahead in winter and three in summer. Check that a video visit confirmation shows your own time zone.

A mainland doctor needs a Hawaii license

The license rule follows the patient. A doctor treating you online while you're home in Hawaii needs a Hawaii license, and a California or Washington license alone doesn't cover it.

There's a narrow exception. A mainland doctor may consult with your Hawaii-licensed doctor, as long as your Hawaii doctor stays in charge of your care.

That matters if you split the year between the islands and the mainland. An office visit on the mainland follows that state's rules, covered in HRT in California, HRT in Washington and HRT in Oregon.

Mainland doctors now have a faster route. Hawaii joined the Interstate Medical Licensure Compact, and compact licenses became available in January 2025. The compact speeds up a Hawaii license for doctors already licensed in another member state.

Hawaii hasn't joined the Nurse Licensure Compact. A nurse practitioner treating you online needs a Hawaii license as well.

Hawaii nurse practitioners prescribe on their own

Hawaii lets advanced practice registered nurses, a group that includes nurse practitioners, diagnose and prescribe under their own license. The American Association of Nurse Practitioners lists Hawaii among its full practice states.

Section 457-8.6 lets the Board of Nursing grant prescriptive authority to qualified nurse practitioners, including for controlled drugs. Estradiol and progesterone aren't controlled, so a Hawaii-licensed nurse practitioner can prescribe them after an online evaluation.

Testosterone has extra rules

Testosterone is a Schedule III drug, a federal class of controlled medicines with stricter handling. The prescriber needs a DEA registration, and Hawaii adds its own state controlled substance registration.

To start it without an in-person exam, prescribers rely on federal telemedicine flexibilities. They run through December 31, 2026. The DEA's permanent rule went to White House review in August 2026 and isn't final yet.

Midi lists a testosterone cream in its online store, from $100 per 90 days. Winona and Evernow don't list testosterone. No testosterone product is FDA-approved for women. A 2019 global consensus statement supports it only for low sexual desire after menopause.

Will HMSA, Kaiser Permanente, Med-QUEST, EUTF or Medicare pay for hormone therapy?

Coverage usually reaches the hormones themselves, but never a telehealth subscription. Visits fall in between, because Midi and Evernow bill some plans for them.

Hawaii stands apart in one way. Under the Prepaid Health Care Act, chapter 393 of the state's laws, most employers must offer health coverage to employees who work at least 20 hours a week.

Coverage starts after four straight weeks of work. Your share of the premium can't top half the cost or 1.5 percent of your monthly gross pay, whichever is less.

Hawaii also has a telehealth law for private plans, in sections 431:10A-116.3, 432:1-601.5 and 432D-23.5. Insurers, HMOs and mutual benefit societies can't require an in-person visit before they pay for telehealth. Through 2027, they must pay the same for a telehealth visit as for the same visit in person.

Your plan's own terms still apply. Self-funded employer plans, common at large national companies, follow a federal law called ERISA instead.

Winona, Alloy, Inner Balance and The HRT Club don't bill insurance, and no plan covers Evernow's membership. Our HRT insurance guide explains how to read a drug list and appeal a denial.

HMSA

HMSA is Hawaii's Blue Cross Blue Shield company. Like every plan, it publishes a formulary, its list of covered drugs sorted into price tiers.

HMSA's Essential Drug Formulary for commercial members took effect October 1, 2026. Generic estradiol tablets, estradiol gel, and weekly and twice-weekly patches all sit on tier 1, the mostly generic tier.

Progesterone capsules sit on tier 1 too, as do vaginal estradiol cream and tablets. None of these lines carries a flag for prior authorization, meaning approval before you fill.

HMSA publishes more than one drug list, so confirm which one your plan uses in your member account. If you're on HMSA's HMO plan, ask before booking an online service. HMSA says its HMO covers mainland providers mainly for emergency or urgent care.

Kaiser Permanente

Kaiser Permanente runs its own clinics, doctors and pharmacies in Hawaii. Its HMO plans generally need a plan referral before they pay an outside provider, except in emergencies.

So a Kaiser member usually starts with a Kaiser doctor or gynecologist, and the prescription runs through Kaiser's own drug coverage. Paying an online service out of pocket remains an option outside the plan.

EUTF, for state and county workers

State and county employees and retirees get coverage through the Hawaii Employer-Union Health Benefits Trust Fund, or EUTF. With an HMSA medical plan, prescriptions come through a separate CVS Caremark drug plan. Kaiser plans include their own drug coverage.

So an EUTF member on an HMSA plan looks up estradiol and progesterone on Caremark's list, not HMSA's.

HealthCare.gov plans for 2027

Hawaii uses HealthCare.gov for individual plans. Open enrollment for 2027 coverage runs November 1, 2026, to January 15, 2027. Pick a plan by December 15 for coverage that starts January 1.

Two insurers sell marketplace plans for 2027, HMSA and Kaiser. HMSA's approved rates rise 13.3 percent before subsidies. Kaiser's rise 9 percent.

Med-QUEST

Med-QUEST is Hawaii's Medicaid program. It covers adults with income up to 138 percent of the federal poverty level.

Hawaii gets its own poverty guideline, set higher than the mainland's. In 2026, the limit works out to about $25,300 a year for a single adult.

Most members join a QUEST health plan. The five are AlohaCare, HMSA, Kaiser Permanente, 'Ohana Health Plan and UnitedHealthcare Community Plan. Not every plan is open on every island.

Each plan keeps its own drug list. HMSA's QUEST list, in effect from November 1, 2026, covers generic estradiol tablets, weekly estradiol patches and progesterone capsules with no prior authorization. Testosterone gel needs approval first.

Big changes start January 1, 2027, under a 2025 federal law. Most expansion adults aged 19 to 64 will need 80 hours a month of work, volunteering or school, or income of at least $580 a month.

Those adults will also renew every six months instead of once a year. Exemptions include pregnancy, disability, being medically frail, and caring for a child under 14 or a disabled person.

The state says the new rules apply to about a third of Med-QUEST members. None of the online services above takes Med-QUEST, so a clinician in your QUEST plan is the realistic route.

TRICARE

Many military families in Hawaii use TRICARE. In 2026, a generic drug on its formulary costs $16 for a 30-day fill at a network pharmacy.

Home delivery costs $14 for up to a 90-day supply. Active-duty members pay nothing.

Medicare

Medicare drug plans, called Part D, set their own drug lists. Your plan's list shows whether estradiol and progesterone are covered, and on which tier. In 2026, your out-of-pocket spending on covered drugs stops at $2,100.

Medicare's open enrollment runs October 15 to December 7, 2026, so there's still time to compare drug lists for next year. Midi treats Medicare as out of network, so visits cost its self-pay rate. Evernow doesn't accept Medicare.

Finding a menopause doctor or menopause specialist in Honolulu, on Maui, the Big Island or Kauai

The Menopause Society's practitioner directory, at portal.menopause.org, is the usual first stop. It lists members and Menopause Society Certified Practitioners, known as MSCPs.

Queen's Health Systems lists menopause management among its women's health services. Its women's health locations include Honolulu, Ewa Beach, Kahului on Maui and Kamuela on Hawaii Island.

In Kona, Lihue or Kaunakakai, run the directory with your own ZIP code. Look for the MSCP mark, which shows a clinician certified in menopause care.

On paper, Hawaii's statewide OB-GYN gap looks small. The state's physician workforce report, released in December 2025, found a statewide gap of about two full-time OB-GYNs.

That gap sits on the neighbor islands. Hawaii County was short 1.5 full-time OB-GYNs, about 7 percent of what it needs. Kauai County was short less than half of one. Honolulu and Maui counties showed no OB-GYN gap.

The wider doctor shortage is the bigger problem. The same report found Hawaii County needed 224 more doctors of all kinds, the largest gap of any county.

If the nearest specialist means an interisland flight or a long wait, online care can be a faster start. A local visit still adds what a screen can't, such as a hands-on pelvic exam.

To size up a local clinician, see our guide to finding perimenopause care. HRT near me helps you weigh an online clinician against the closest office, and finding an HRT doctor covers what to ask.

Bioidentical hormones, compounded creams and pellets in Hawaii

Bioidentical describes the shape of a hormone, not how it's made. The molecule matches the estradiol or progesterone your ovaries produce. Most estradiol patches and micronized progesterone capsules meet that test, and they're FDA-approved too.

Compounded hormones are mixed by a pharmacy for one patient and never go through FDA approval. Hawaii's Board of Pharmacy requires any out-of-state pharmacy that mails prescription drugs into Hawaii to hold a Hawaii permit, called a miscellaneous permit.

Pellets are small compounded hormone implants placed under the skin every few months. Biote's provider directory has pages for Honolulu, Ewa Beach, Waialua and Kahului.

Expect to pay cash for pellets, and ask for a written quote that covers the pellets, insertion and labs. Read our hormone pellets guide before you book.

At the FDA's request, the National Academies reviewed compounded bioidentical hormones in 2020. They found little evidence of safety or benefit. ACOG and The Menopause Society both prefer an FDA-approved product whenever one exists.

Approved products are getting new safety labels. In February 2026, the FDA approved revised labels for a first group of six hormone products. Statements on heart disease, breast cancer and probable dementia came out of the boxed warning, a label's most prominent caution.

Three estradiol patches followed in late October 2026, with the same statements removed.

In the table, Winona is mixed: its patch, tablets and capsules are FDA-approved, while its creams are compounded. Alloy's hormone protocol, Midi's default treatments, The HRT Club's drugs and the coupon generics are FDA-approved. Inner Balance's Oestra cream is compounded, and Evernow is mostly approved with a few compounded items.

If you want compounded hormones on purpose, compounded HRT: where to get it covers what to check.

Starting HRT in your 40s in Hawaii: labs, age and the questions to ask

Plenty of women in their 40s hear that they're too young for hormones. The Menopause Society's 2022 position statement says benefits outweigh risks for most healthy women with bothersome symptoms. That holds under age 60, or within ten years of menopause, unless something rules hormones out, such as a past blood clot or stroke.

Perimenopause comes before the last period, so it falls inside that window. Hormone levels swing too much in these years for a blood test to confirm it. Winona and Alloy prescribe without required labs, and labs are optional at Inner Balance.

The same holds if you have no periods to track, after a hysterectomy or with a hormonal IUD. Your symptoms guide the conversation. Perimenopause labs covers what a hormone panel can tell you, and what it can't.

Before you commit, put these four questions to any prescriber who treats you in Hawaii, on a screen or in an exam room.

  • License: Do you hold a Hawaii license, or does a Hawaii-licensed clinician oversee my care?
  • Training: Have you trained in menopause care, or earned The Menopause Society's certification?
  • First choice: Do you begin with FDA-approved estradiol and micronized progesterone, and turn to compounded hormones only if those don't suit me?
  • Refills: Will my medicine ship to my island address, or can you send it to my local pharmacy?

Once you start, our HRT dosage guide walks through how the dose gets adjusted.

Frequently asked questions

Is it legal to get HRT online in Hawaii without an in-person visit?+

Yes. Section 453-1.3 of the Hawaii Revised Statutes lets a physician establish a physician-patient relationship through telehealth, as long as the physician holds a Hawaii license. Telehealth includes video and secure messages reviewed later. The same section says a prescription based solely on an online questionnaire is not treatment and doesn't meet the standard of care. Expect a real exchange with a clinician. Midi and Evernow offer video visits.

Can online HRT services ship to Maui, Kauai, Molokai or the Big Island?+

Winona lists Hawaii among its states, and Alloy, Midi and Evernow say they serve all 50 states, neighbor islands included. Hawaii requires out-of-state pharmacies that mail prescriptions into the state to hold a Hawaii permit. If you'd rather pick up locally, Winona's and Alloy's terms let you send your prescription to a pharmacy you choose.

Do HMSA and Kaiser Permanente cover HRT in Hawaii?+

HMSA's Essential Drug Formulary for commercial members, in effect from October 1, 2026, puts generic estradiol tablets, gel and patches, plus progesterone capsules, on tier 1 with no prior authorization flag. Your plan may use another list. Kaiser covers prescriptions through its own clinics and drug coverage, and its HMO plans generally need a referral before paying an outside provider. Neither covers a telehealth membership. Hawaii law bars plans from requiring an in-person visit before paying for telehealth.

Does Med-QUEST cover HRT, and what changes in 2027?+

Med-QUEST covers adults with income up to 138 percent of the federal poverty level, about $25,300 a year for a single adult in 2026. Each QUEST plan keeps its own drug list. HMSA's QUEST list covers generic estradiol tablets, weekly patches and progesterone capsules without prior authorization. From January 1, 2027, most expansion adults aged 19 to 64 will need 80 hours a month of work, volunteering or school, unless exempt, and will renew every six months. None of the online services on this page takes Med-QUEST.

Can a Hawaii nurse practitioner prescribe HRT without a doctor?+

Yes. Hawaii lets nurse practitioners diagnose and prescribe under their own license, and the American Association of Nurse Practitioners lists Hawaii as a full practice state. Section 457-8.6 lets the Board of Nursing grant prescriptive authority to qualified nurse practitioners. Estradiol and progesterone aren't controlled drugs. For testosterone, a Schedule III drug, the prescriber also needs a DEA registration.

My mainland doctor prescribes my HRT. Can she keep treating me online after I move or while I winter in Hawaii?+

Only with a Hawaii license. Hawaii's telehealth law ties the physician-patient relationship to a license to practice medicine in the state. A mainland doctor without one may still consult with your Hawaii-licensed doctor, as long as the Hawaii doctor stays in charge of your care. Hawaii joined the Interstate Medical Licensure Compact, which speeds up a Hawaii license for doctors licensed in another member state. When you're back on the mainland, that state's rules apply.
